Medication reminders, prescribed activity and simple rewards for people ageing at home.

This is a working prototype of a community care engagement tool. It is not a live service, not a medical device, and not connected to any hospital system.

What MediKaki does

Medication reminders

Simple, timed nudges for the medicines the care team prescribed. A log is kept for the person and their authorised carer.

Prescribed activity

Exercises, balance work and light movement prescribed by a physiotherapist or doctor. Progress is tracked, not guessed.

Small rewards

Crystals and vouchers for consistent engagement. Rewards come from the programme budget, not from public funds or the hospital.

Recognition, not payment

MediKaki recognises effort and attendance. It does not pay people to take medicine or replace clinical judgement.

How it works

Enrol

A nurse or doctor invites the person and their carer. Consent is captured before any data is shared.

Prescribe

The care team sets medicines, activities and goals. Everything is tied to a real role and recorded in the audit trail.

Engage

The person gets reminders, marks doses done, and logs activity. Caregivers see only what the person consented to share.

Monitor

Risk flags, missed doses and stale activity surface to the right staff member. Escalation stays inside the care team.

Built-in commitments

  • No NRIC, no real identifiers, and no linkage to any national system in this prototype.
  • Three separate consents: care data sharing, caregiver visibility, and reward programme participation.
  • Rewards are funded by the programme budget, not by the hospital or any public scheme.
  • MediKaki is for engagement and workflow support. It is not for emergencies. For emergencies call 995 or your usual helpline.

Available languages

Patient and caregiver surfaces can be shown in the four official languages. Translations in this prototype are machine-generated and marked for human review before any pilot.

What this prototype is

Real in this prototype

  • Role-based access and the workflows it enables are genuinely enforced by the database.
  • Who may prescribe, verify, sign, enrol or break glass follows the same rules the product would ship with.
  • Actions that change a record are written to an append-only audit trail.
  • Consent, rewards, medication scheduling and escalation rules are implemented as working logic.
  • The four-language patient and caregiver experience is functional, with machine translations flagged for review.

Not real in this prototype

  • Every person, medication, care plan and message is invented. No real patient data exists here.
  • No hospital system is connected. There is no live pilot, no approved deployment and no production data flow.
  • Clinical content, contraindications and progression rules are illustrative and have not been clinically reviewed.
  • Rewards, crystal values, vouchers and any grant references are placeholders pending programme sign-off.
  • Consent wording is a draft and would need legal and ethics review before use with real people.
